At Cockburn Veterinary Group we wish to provide the best and most appropriate treatment for your pets. In order to facilitate this we need to maintain records of your name, address, telephone numbers and email address alongside their clinical notes. To ensure your data is stored safely we back it up to ISO 27001:2013 compliant cloud. We do not sell, rent or trade email lists with other companies. The data will be collected through verbal communication, in writing, or via our website portal when you create an account and will be held for a minimum of 6 years.
We wish to use the data primarily to send you appointment reminders; preventative health care reminders for example for vaccinations, worm and flea treatments; or on occasions for new services or products. We would also like to contact you to inform you that a requested prescription is ready for collection or to provide details about your pet’s treatment whilst under our care. At any time you can opt out of receiving this information by contacting the practice either in person, by telephone on 01530 836654, or by emailing email@example.com.
We will on occasion share your data with referral veterinary practices when, after asking your permission, we wish to refer you pet for them for further examination or ask their advice regarding your pet’s health. We will also need to share your data with external laboratories in order to identify the samples being submitted to them; with insurance companies when submitting an insurance claim on your behalf; and after insertion of a microchip with centralised microchip data logs so that should your pet become lost you can be contacted.
In the event of another veterinary practice telephoning for a copy of your records, we will attempt, where possible, to contact you for permission prior to releasing details.
If you telephone for assistance outside normal working hours your call will be directed to an answering service. These telephone calls will be recorded for training and to ensure details have been collected correctly. This data will be erased after 3 months.
